O: Checked vitals. Noted some slight lung sounds and bird was popping air sac. No arrhythmia noted. Removed R figure 8 wrap/body band. Performed PT to R wing, noted R shoulder stiffness and palpated grinding of bone at fracture site. Placed R figure 8/body band. Gave oral meds. Dropped 40g of rat. Noted 1 cast.

O: Checked vitals, lungs clear. Noted heart arrhythmia were heart rate skipped 2 beats spread out from one another. R figure 8 wrap/body band intact. Anisocoria still present. Gave oral meds. Dropped 44g of rat. No regurge and crop empty.

S: On perch, jumped to back. Seemed to be leaning to the L a little. Normal feces in cage.

O: No anisocoria noted today, heart and lung sounds WNL. Bird acting visual OU, very mild debris present in the vitreous OD. Small bruise on the dorsal R carpus noted during wrap change. Creptius still present in the R shoulder on palpation. Good extension in wing during PT. Replaced figure-8 and body wrap. Small amount of blood seen around the glottis. Bird also seemed to lean to the L wing placed back in cage.

O: Collected fecal sample from cage. Noted a lot of urates/feces smeared around incubator. Somewhat grainy. Took temp and listened to heart/lungs. No abnormalities on auscultation. Placed tail wrap. R wing wrap and body band in place and didn't need adjustments. Gave 43 cc LRS SQ in L leg inguinal area. No notable anisocoria while in hand. Gave meds PO. Moved to crate off of oxygen. Dropped 40 g of venison.

O: Checked vitals. No feces to collect. Collected blood from R medial metatarsal. Removed R figure 8/body band. Took rads. Placed on ground in exam room. Noted deep pain response in both toes and legs. Placed R figure 8/body band. No stain uptake OU. Gave fluids and oral meds. Dropped 42g of whole chick.

A: Confirmed R coracoid and scapula fractures on x-rays. No spinal fracture appreciated on rads, so likely that patient isn't standing on it's own due to head trauma. Has deep pain response in legs and was trying to use them when placed on the ground. Vital signs and mentation were improved compared to intake. Will continue to monitor condition.

P: Try off of O2 and move to crate tomorrow if stable. Retry fecal collection. Monitor leg use and fecal output. Start on tramadol.

Initial treatment: 0.18cc Torb; 0.15cc Simbadol; 0.29cc Meloxicam.

A: Patient's mentation was poor and vitals were on the quiet side, so opted to skip intake rads and get on oxygen support quickly after exam. Patient suffering from R shoulder fracture, feels like scapula and coracoid fracture. Also noted head trauma based on depressed mentation and anisocoria. OD hemorrhage is very minor on the lateral aspect of pectin. Will take rads once stable to further assess shoulder fracture.

P: Rx meloxicam, calcium slurry, fluids. Start on Tramadol tomorrow evening. Monitor all vitals. Take rads 12/5 if stable. Collect blood and fecal. Monitor mentation. Stain eyes 12/5. Monitor anisocoria. Change R figure 8/body band E3D. Vet check 12/8.
